The Leave From Work Form must be completed whenever a staff
member has been/will be absent from work. This form is a
PDF and will require the user have Acrobat Reader.
Instructions:
1. Fill in the appropriate blanks on the form
(or print the form and fill it out manually).
2. Print out the form (print TWO copies of you want one
returned to you).
3. Sign the form
4. If you are a Research Tech, please have the form signed
by your supervisor before submitting to the Botany Office.
5. Submit to Aven or Sarah.
If two copies were submitted, one will be returned
to you for your records.
